1998 Mustang GT Phantom Code Issue.

I have been having issues with my GT as of late. At first, my car would suffer a loss of HP from a stop, you'd give it a little gas, and you could hear the engine try to react, but you'd get nearly no output from the engine for a hand-full of seconds, and then it would take off like a bat out of Hell, before giving you the output you wanted in the first place.
